Copays and deductibles are due at the time of service.
Outstanding balances must be paid in full before scheduling new services or completing paperwork.
Completion of FMLA, disability, or insurance paperwork requires a thorough review of records and is not included in regular session fees.
Standard fee: $100 for 1–6 pages (discounts may be applied at the provider’s discretion).
Payment is required before paperwork is processed. Please allow 7–10 business days for completion.
Patients requesting disability paperwork must be seen consistently for a minimum of 6 months or at least 6 visits before such paperwork can be completed.
All visits must be scheduled in advance; walk-ins are not accepted.
Appointments must be canceled at least 24 hours in advance.
Cancellations made less than 24 hours before the appointment will be charged $50.
No-shows or same-day cancellations will be charged $75.
